I'm over 18 and taking the 24hr Driver's Ed Course, do I need to fill out the parent affidavit?
OHIO DPS Rules: The State of Ohio requires that students verify their identity before the course can be started if they are 18 or older.
If students are under 18, we must verify the identity of their parent or guardian.

If you are 18+ and need to submit an affidavit for the 24hr Course:
- You still need to submit the Parent/Guardian Affidavit. (Download it here)
- You can enter your own information on the form OR have your parent/guardian complete the form.
- If you use your own information on the form, make sure to write on the affidavit: "Student over 18" or we may not approve the form when it comes in.
If you choose to complete the affidavit with your details, your name will be listed as both the parent and child.
Remember to write "Student over 18" on the form.
