- Parents who do not hold a license can write on the form "No license" or enter a state ID number (if they have one).
Parents/guardians who are uncomfortable entering details due to immigration/VISA status:
- They do not need to enter details in the Out of Country/International section of the form -- it is optional.
The most important part of the Parent/Guardian affidavit is that:
- The parent/guardian name and license matches with what is entered on the student's course contract
- The student's name matches what is on the account
- That the form itself is notarized

If there are extenuating circumstances where someone else can be considered a guardian for the student, then it would also make sense for them to be able to complete the form.
