My parent doesn't have a license or doesn't want to put their information on the affidavit, what can I do?

For the parent affidavit, it asks for a parent's driver's license number.  If you have a parent that does not drive or who does not want to disclose personal information due to immigration/visa issues, here is some important information.
Parents/guardians without a license:
  1. Parents who do not hold a license can write on the form "No license" or enter a state ID number (if they have one).
Parents/guardians who are uncomfortable entering details due to immigration/VISA status:
  1. They do not need to enter details in the Out of Country/International section of the form -- it is optional. 
The most important part of the Parent/Guardian affidavit is that:
  1. The parent/guardian name and license matches with what is entered on the student's course contract
  2. The student's name matches what is on the account
  3. That the form itself is notarized
If there are extenuating circumstances where someone else can be considered a guardian for the student, then it would also make sense for them to be able to complete the form.
