Form:  50hr Affidavit: BMV 5791

For first-time drivers in Ohio who are under age 21, 50 hours of supervised driving are required. You must drive with a licensed driver 21 years of age or older. Also, Ohio BMV will require you to provide a completed and notarized Fifty-Hour Affidavit (form BMV 5791). You will be bringing this form to your road test.
